Wayne Elementary will be having a COVID Pfizer vaccination clinic on 11/19/21, with second doses 12/10/21. We will be partnering with Northern Light Home Care & Hospice to provide this clinic to our students. This clinic is an opt-in clinic and students must be registered to participate.
Please see the attached letter from Northern Light with all the information regarding the clinic. Northern Light is asking for an electronic registration here is the link: Wayne Elementary COVID Vaccine Consent Form but the office does have forms if you are unable to access it electronically. Please register as soon as possible if you are interested as this is an opt-in clinic, if you have any questions please let me know. I am so excited that we are able to offer this to our students in Wayne.
